Forms

Make a form, share a link, and every answer lands as a row in a database on your own server. Contact forms, RSVPs, feedback, job applications, orders and sign-up sheets, with no account needed to fill them in.

A form is a file in your Drive, like a Doc or a Database. You build it in the browser the way it will look, and people answer at a short link like drive.example.com/f/team-lunch. The answers go into a Stavo Database next to the form, with a column per question, so every view, filter and export works on them.

Make a form

  1. In the Drive, click New > Form.
  2. Pick a template (Contact, RSVP, Feedback, Job application, Order form or Sign-up sheet), or start from scratch.
  3. Type a title and a description, then add questions with the + between them.

Everything saves as you type, and two people can build one form together. History and Restore work like in every other editor.

Question types

  • Short answer and Long answer, with a length limit and, for short answers, a pattern to match.
  • Email, Number (with a smallest and biggest), Phone and Link.
  • Single choice, Multiple choice (with how many to pick) and Dropdown.
  • Rating (3 to 10 stars) and Scale (0 or 1 to 10, with words under the ends).
  • Date (with an earliest and latest day), Time and Yes or no.
  • File upload, with how many files, how big and which types, like .pdf or image/*.
  • Signature, drawn with a finger, pen or mouse. It's saved as a picture.
  • New page splits the form into steps, with a progress bar.
  • Hidden field, filled from the link, like ?ref=newsletter.

Mark a question Required from the bar that shows when you're on it. Every answer is checked on the server too, so the rules hold even for people who skip the page.

Logic

Open a question's settings (the gear) to make the form react to answers:

  • Show this question only when earlier answers match, like "Show the address when Delivery is Ship to me".
  • After this page, skip ahead to a later page or straight to the end, like "skip the meat questions when the answer is Vegetarian".

Questions that aren't shown aren't required, and what was typed in them before is left out.

After sending

The card at the bottom sets the thank-you screen: a title, a message, and an optional page to send people to afterwards. You can also rename the Send button.

Design

Click Design under the title for an accent color, a font (Sans, Serif, Rounded or Mono) and a background picture, and Add cover for a picture on top. The link preview in chats uses the form's title and accent.

Sharing

The Publish tab has the link and who can answer:

  • Custom link: pick the end of the address, like /f/team-lunch.
  • Access: anyone with the link, people signed in to your server, or only people you pick by username. You can add a password to any of them.
  • Opens and Closes dates, a response limit, and an Accepting responses switch.
  • Responses per person: as many as they like, one per person (they sign in first), or one per device.

Spam is kept out without a CAPTCHA: a hidden field only bots fill in, a check that the form wasn't sent faster than a person could, and limits on how often one address can send.

Put a form on your website

The Publish tab has the embed code. Paste it into any page:

html
<iframe src="https://drive.example.com/f/team-lunch?embed=1" width="100%" height="720" style="border:0;max-width:720px" loading="lazy"></iframe>

Embedded forms work on any site. Password forms ask for the password inside the frame. Forms for signed-in people work best as a plain link, since browsers often block sign-in cookies inside frames on other sites.

Responses

  • Responses shows the form's database, live. Add views, filters, sorts and notes on each row like in any database. Export CSV downloads the answers with a column per question.
  • Summary shows a chart per question, averages for ratings and scales, recent text answers, and responses per day.
  • Uploaded files and signatures are private files in a folder next to the form, linked from each row. They only reach your Drive when someone sends the form.
  • Want the answers in a database you already have? Publish > Use another database. Columns are matched by name, and missing ones are added.

Notifications

  • You get a notification for new responses, folded into one until you look, and an email when email is set up on your server.
  • Prefer quiet? Switch to a daily digest, or turn it off.
  • Email people their answers sends a copy to whoever filled in the form, when your server has email set up and the form has an Email question.

Keyboard

  • Mod+Enter adds a question below the one you're on. Type to filter the question types, Enter to pick.
  • Alt+Up and Alt+Down move a question, Mod+D duplicates it, Mod+Shift+Backspace removes it (with Undo).
  • In choices, Enter adds the next one and Backspace on an empty one removes it.
  • People filling in a form can press a letter to pick a choice and Mod+Enter to go on.

Mod is Command on a Mac and Ctrl everywhere else.

Ask the Assistant

The Assistant and Claude (over MCP) can list your forms and read their responses, like "summarize this week's feedback form answers". Since the answers are a database, they can also filter them.

For admins

Forms can be turned off, or given to selected people only, in Admin > Features. Forms need Databases. Turning Forms off hides the builder and every form link; forms and their answers stay where they are.
